Service Description

Whether you're a multi specialty practice or a primary care provider we can help you provide monthly phone calls to enrolled clients and integrate personalized patient assessments from each call into an EMR system so the provider can be a part of the chronic care management process.

Cancellation Policy

To cancel or reschedule an appointment please allow 3 business days in advance. Send an email to nursetoy@angelsconciergeservice.com or text me at 323-421-0103. If Angels Concierge Service needs to cancel or reschedule we will give at least 2 day notice to the client and /or facility.
